What you’ll need:

Proof of insurance

Minimum $100,000 cargo insurance
Minimum $1,000,000 auto liability
Insurer with B+ or better A.M Best Rating

Details about your modes, commodities, tractor/equipment counts and certification

Operating authority with the below requirements

Valid interstate or intrastate authority (intrastate-only carrier must also have an active U.S. DOT #)
Satisfactory or NOT rated by the DOT
If NOT rated by the DOT, you will be subject to additional qualification procedures
Must be in business for 9 months

Completed electronic Master Transportation Contract

Completed W-9 Form for tax purposes
